Building healthier communities
across NYC
Every New Yorker deserves the opportunity to live a long, healthy life.
We serve families, fund community partners, and lead a city-wide network of social service providers to break down barriers to care.
How We Help
Connecting care and community to help New Yorkers thrive
We work across systems to close gaps and create a healthier, more equitable New York City.
Are you enrolled in Medicaid?
WholeYouNYC connects Medicaid members in Manhattan, Brooklyn, and Queens to critical services—such as food, housing, transportation, and health insurance—through a network of more than 300 community-based organizations.
Are you pregnant or a parent?
We connect expecting moms and parents in all five boroughs to essential services and resources.
Do you need food assistance?
We help enroll people in WIC and SNAP benefits so New Yorkers can overcome food insecurity.
Are you looking for health insurance?
Our health insurance navigators help people and families get insurance through New York State and provide specialized support for seniors and people with disabilities.
Do you live in NYCHA Housing in Harlem?
Our community-based Harlem Health Advocacy Partners (HHAP) program improves the health and neighborhood conditions of NYCHA residents in East and Central Harlem.
